WHAT YOU WILL NEED TO BRING:
- Current income jof all family members and applicatgion (a copy of last year's Fedral Income Tax return, W-2 form, two current pay stubs, etc.
- Legal residency or immigration documents (if applicable - the numbers on the card are required in the application.
- Copy of SSN (only if your are applying for coverage) and DOB for each family member in the household
OPEN ENROLLMENT ENDS January 31, 2016. Most people are required to have health insurance or pay a penalty on their tax returns. Tax penalties in 2016 will e higher than the year before.
